PATIENT INFORMATION
WELCOME
Welcome to our Web Page. Our primary purpose is to provide you with
the highest quality obstetrical and gynecologic care in a pleasant surrounding
with courtesy and understanding. We do want your comments as to how we
may better satisfy your medical needs. Please feel free to contact us.
LOCATION
Our main office is located in downtown St. Paul across from United
and Children's Hospital in the Ritchie Medical Plaza. All specialty procedures
(colposcopy, laser surgery, ultrasound, and non-stress testing) are done
at this location. For your convenience we also operate a satellite clinic
in Eagan. Be sure to specify which location at which you wish to
be seen.
Most surgery is done at United or St. Joseph's Hospitals.
APPOINTMENTS
For routine health care and non-emergent problems, appointments should
be made approximately 2 to 3 weeks in advance. If you have an urgent problem,
please inform the receptionist of this and we will see you as soon as possible.
We attempt to see patients with bladder or vaginal infections the same
day whenever possible. When making an appointment, please tell the receptionist
the nature of the problem so that the appropriate amount of time can be
scheduled for your visit. Because of the nature of our specialty practice,
occasionally, a delay will be unavoidable. When possible, we will call
you if we are behind schedule. If you find that you cannot keep your appointment,
we would appreciate if you would call the office as much ahead of time
as possible. This will make it possible for another patient to be seen.
TELEPHONE CALLS
If you have any questions or concerns, please call our office in St.
Paul. Our office staff is trained to answer many of your questions. Consultation
with your Nurse Practitioner or Doctor will be made when necessary. Your
phone call will be returned at the earliest opportunity available. At times,
your return call may be after office hours when we are no longer seeing
patients. We request that all prescriptions are refilled during normal
office hours. When you call, please have the number of your pharmacy
available. In an emergency, your Doctor can be reached by calling 651
244-4897 (St. Paul office). Our answering service will take your name and
telephone number and relay the message to the on-call Doctor and he will
return your call as soon as possible.
If you have caller ID that blocks anomonous calls, please deactivate
it while waiting for a call back as some of the doctors covering do not
want their cell phone number in the public areana. Thank you for your cooperation.
If, for any reason, you cannot reach the office (power outage, etc.)
You can reach your doctor by calling United Hospital Labor & Delivery
at
651 220-6220 or the Emergency Room at 651 220-8755. Explain
that our line is not being answered and ask them to page the oncall physician.
MEDICAL RECORDS AND CONFIDENTIALITY
All your medical records are confidential. We will not release your
record or any part of your medical record without your written authorization.
Information will not be released to your family, insurance company, or
other physicians without your consent.
FEES AND BILLING
Every effort is being made to keep your health care costs down. Our
fees are within the customary range for this area and reflect the high
quality care you will receive. If you have any questions regarding our
fees or billing, please feel free to discuss this with our business manager.
If any financial problems arise, please inform us as soon as possible so
that we may work with you in meeting your financial commitments. Itemized
statements are mailed monthly, after the initial insurance claim has been
filed. Outstanding balances should be paid within 30 days.
INSURANCE
As a service to you, our office will assist you with insurance forms.
Please let us know the type of insurance you have and the group and policy
number. Please bring your insurance card to each visit.
| Please remember that insurance
is considered a method of reimbursing the patient for fees paid to the
doctor and is not a substitute for payment. Some companies pay fixed allowances
for certain procedures and others pay a percentage of the charge. It is
your responsibility to pay any balance not paid for by your insurance company.
Questions can arise on insurance
matters and these should be discussed with our office mainager. We will
be happy to help you receive maximum benefits; howerver, the agreement
of the insurance company to pay for medical care is a contract between
you and the company. |
